If you wish to receive reward checks, an updated W9 (Rev. October 2018) is required. Download a W9 form and upload it to your completed rewards application below. You can also email completed applications to [email protected].

If you wish to receive credits on your account, a Norfolk charge account must first be established. If you do not have an account, please fill out the Norfolk charge account application and upload it to your completed rewards application below. You can also email completed charge applications to [email protected].

The enrolled Rewards Member agrees to inform their designer when they are sending in a customer, and will provide their name and contact information in advance. Rewards Members will receive their reward within 45 days after project completion. Previous purchases are not eligible for rewards. Norfolk Kitchen & Bath shall not be construed to create in any manner an employer-employee, principal-agent, joint venture or partnership relationship between the parties. All parties will indemnify and hold the other harmless from and against any claims, actions, liabilities, damages, costs, and expenses arising out of this agreement. Norfolk Kitchen & Bath reserves the right to change or terminate this rewards agreement without notice.
